Personal health budgets(1) (PHBs) are transforming the lives of patients suffering from long-term conditions. PHBs are sums of money, paid by the NHS(2), which allow patients to help design a flexible, personalized package of care support from clinicians and others.

Anita(1), 57, has Huntington’s disease and is on a PHB pilot scheme. Her PHB, developed and implemented by her NHS care navigator(3) in full consultation with Anita’s husband, Trevor, currently provides seven Personal assistants (PAs) who care for her. The flexibility of the PHB allows them to give her medical and personal care as well as helping her to access the community through trips to the shops and the theatre.

Anita’s feeds are administered by her carers through a gastrostomy tube. A gastrology nurse, who changes the tube or PEG every 2-3 months, has trained Trevor to change the peg in case of an emergency, thus avoiding a crisis hospitalisation. The nurse has been instrumental in enabling Trevor to be confident and competent in performing this procedure, which he has needed to do three times in past two years.

The PHB roll out(4) will mean that by April 2014 up to 56,000 people will be eligible to ask for one. The money will buy PAs, Wiis, leisure passes, home training and delivery of medical supplies amongst other benefits.

Patients will gain independence, wellbeing and support in their family context. Financial officers are happy too : the number of « bed-blockers » is significantly reduced and care is directed where it is most needed.
